Position Summary Independent Optometrist – Sam’s Club Launch your services in Sam’s Clubs by leasing space inside our stores. As an Independent Optometrist, you will take a leading role in patient care with respect to eye and vision care, as well as general health and well‑being. Responsibilities Establish your days of practice and hours of operation. Control your patient schedule. Set your fee schedule and keep 100% of exam/professional fees. Provide high‑quality eye and vision care to guests. Lease Arrangement and Options Equipments/Instrumentation as needed for patient care. Administrative and/or Opt‑Tech support services (if state allows). Access to fair market value terms and services offered by Gumberg Asset Management Company for ME & VA license agreements. Minimum Qualifications Doctor of Optometry (OD) from an accredited optometry school. Valid state optometry license. Ophthalmologist (MD or DO) from an accredited school. Valid state ophthalmology license. General Liability Certificate. Professional Liability Insurance Certificate. W‑9 form. Additional Information Position is full‑time, entry level, and located at one of the following Sam’s Club locations: 3900 Airline Dr, Metairie, LA 70001‑5704, United States of America 455 31St St, Kenner, LA 70065‑4101, United States of America 1527 Manhattan Blvd, Harvey, LA 70058‑3405, United States of America Estimated salary: $120,000.00 – $160,000.00 per year.
