Job Overview Department: Wake Forest Baptist Medical Center - Nursing: Care Coordination Position: Case Manager Type: Full time; Hours per week: 40. Schedule: Monday‑Friday 40‑hour work week. Pay Range: $38.20‑$57.30 per hour. Responsibilities Utilize nursing process to develop, implement, and evaluate case management outcomes for defined patient populations. Collaborate with multidisciplinary team including physicians, nurses, rehabilitative therapies, pharmacy, and other disciplines to ensure continuity of care and effective communication. Advocate for patients and coordinate appropriate healthcare services. Educate, consult, and assist in the management of patient population to achieve high quality outcomes cost‑effectively. Analyze patient care using evidence‑based scientific approach to identify opportunities for improvement and cost reduction. Participate in daily case management team meetings (Mon‑Fri) and remain available off‑campus via pager, digital phone, and email. Qualifications Graduate of an accredited School of Registered Professional Nurses; BSN preferred, MSN encouraged. Minimum of five years’ experience caring for the assigned patient population; case management and utilization review experience encouraged. Current RN registration in the applicable state. Case Management certification encouraged.
