Work Shift Day (United States of America) Job Summary: The physical therapist evaluates, plans, and implements rehab programs to enhance mobility, alleviate pain, boost strength, and prevent or correct deformities. Services are delivered cost-effectively, with a focus on quality and customer satisfaction, while adhering to all regulatory standards. This Position Offers Opportunities Across Multiple Locations! Core Responsibilities and Essential Functions: Patient Assessment: Collaborate with physicians to develop, evaluate, and update treatment programs, care plans, goals, and discharge plans based on the patient’s physical therapy needs. Create and adjust care plans with input from the patient and/or family. Develop realistic discharge plans incorporating patient and family goals. Treatment Planning and Provision of Care: Implement and adjust physical therapy treatment plans. Select appropriate activities to help patients achieve their goals. Revise goals and plans with input from patients and families. Provide ongoing education and training to patients and families. Give discharge instructions and referrals to community resources as needed. Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment. Documentation: Document evaluations, treatment plans, and each session according to departmental guidelines. Complete and submit all required documentation, including family education and discharge information. General Requirements: Uphold ethical standards and demonstrate integrity and honesty. Required Minimum Education: Graduate from an accredited physical therapy program Required and Bachelor‘s Degree in Physical Therapy Required or Master‘s Degree in Physical Therapy Required or Doctorate in Physical Therapy Required Required Minimum License(s) and Certification(s): Physical Therapist 1.00 Required Basic Life Support 2.00 Required Required Minimum Skills: Effective communication skills, customer service focused, team oriented, and basic computer skills.
