Job Summary Position Title: Physical Therapist (Outpatient Rehabilitation) at Aurora St. Luke's Medical Center. Status: Part‑time. Hours: 20 per week (Tuesday 7‑3, Thursday 7‑12, Friday 7‑3). Sign‑on bonus: $2,500. Pay range: $41.10 – $61.65 per hour. Responsibilities Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations using appropriate methods. Establish and implement individualized care plans based on evaluation results and functional goals, adjusting treatment plans as necessary. Monitor and evaluate outcomes, provide status updates to physicians and teammates, and document all evaluations, progress notes, team reports, and discharge summaries according to department policy. Plan and complete patient discharge, providing home programs, teaching, referrals, and equipment orders. Communicate and collaborate with stakeholders to optimize patient care. Maintain documentation, daily statistics, billing, scheduling, and application of insurance guidelines for authorizations and visit limits. Maintain site productivity standards and adhere to billing and documentation policies. Clean up after patient treatment, put away equipment, and maintain a neat area. Perform manual techniques, patient positioning, transferring, and transporting duties with proper lifting technique. Demonstrate skill with modalities, exercise equipment, and other devices; understand indications and contraindications. Promote patient safety, follow safety initiatives and infection control measures. Participate in performance improvement activities, coverage planning, and professional development. Orient new staff, provide feedback on peer evaluations, and supervise assistants, extenders, or students as applicable. Minimum Job Requirements Doctorate degree in Physical Therapy from an accredited institution or equivalent. Licensed Physical Therapist in the state of employment. Basic Life Support (BLS) certification for healthcare providers (obtained within 90 days of hire unless otherwise exempt). Intermediate computer skills; excellent communication, interpersonal, and organizational skills. Demonstrated decision‑making and problem‑solving ability; completion of all competency assessments. Annual educational requirements for hospital‑based certifications (e.g., DNV, CARF). Knowledge of growth & development principles and ability to adapt care to patient age‑specific needs. Preferred Job Requirements Advanced certifications related to rehabilitation specialties. Experience supervising physical therapy assistants, extenders, or students. Experience in outpatient rehabilitation settings. Physical Requirements & Working Conditions Must sit, stand, walk, lift, and squat throughout the day; lift up to 50 lbs from floor to waist, up to 10 lbs from waist to overhead, and carry up to 40 lbs at waist height. Push/pull with 30 lbs of force; perform sliding transfer of 150 lbs with second person. Repetitive hand use: simple grasping 5‑15 lbs (20% of workday), pushing/pulling up to 75‑100 lbs (10% of day), fine manipulation during massage and joint mobilization. Functional use of senses for effective communication. Exposure to electrical/chemical hazards, blood and body fluids; must wear protective clothing. May require travel involving road and weather hazards. Operate all equipment necessary to perform the job.
