Therapist - Occupational Therapist

Job Description

Job Summary:
Seeking a qualified Occupational Therapist for home health services, providing patient-centered care in a community-based setting. The therapist will evaluate, plan, and implement treatment plans to improve patient function and independence.

Responsibilities:

• Assess patients' physical, emotional, and cognitive conditions to develop personalized treatment plans
• Implement therapeutic interventions to enhance patient mobility, strength, and daily living skills
• Document patient progress accurately and in compliance with regulatory standards
• Communicate effectively with patients, families, and healthcare teams regarding treatment goals and progress
• Maintain current licensure and certifications, including CPR/BLS, in applicable states of practice
• Ensure patient safety and adhere to all health and safety protocols during home visits

Qualifications:

• Hold a Master's degree or Doctor of Occupational Therapy (OTD), with licensing in relevant states
• Possess valid Occupational Therapy licensure in all applicable states
• Have current CPR/BLS certification from an approved provider
• Maintain a valid Driver's License and be at least 21 years of age or older
• Ideally, have experience with Epic electronic medical records system
• Demonstrate excellent communication and interpersonal skills
