Schedule:

• 13-week assignment, with potential for extension
• Full-time, Monday through Friday
• On-call coverage may be required

Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• Strong OASIS documentation skills
• Proficiency with HomeCare HomeBase preferred
• Reliable transportation and valid driver's license
• Ability to manage structured daily patient visit expectations

Key Responsibilities:

• Deliver skilled in-home nursing interventions, including wound care and medication management
• Complete accurate, timely patient assessments and OASIS documentation
• Coordinate care and collaborate closely with physicians and interdisciplinary teams
• Provide patient education and disease monitoring to promote optimal health outcomes
• Communicate updates and changes in patient status to the clinical team efficiently
