Social Services Consultant

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. This role involves case management, care coordination, social work, and disability services to help participants maintain independence, achieve personal goals, and exercise self-direction. Key responsibilities include developing and updating individualized support plans, conducting regular participant contacts and visits, promoting community inclusion, monitoring health and safety, and educating participants on program requirements and resources. The position requires collaboration with community partners, accurate documentation, and independent work within a supportive team environment. Qualifications include a Bachelor's degree in related fields and experience working with individuals with disabilities or older adults, or equivalent experience. Preferred skills encompass case management, care coordination, healthcare navigation, and cultural competence.
