The role involves providing stepped‑care consults, establishing Self‑Management Goals (SMGs), and conducting follow‑ups aligned with PCP visit schedules or patient needs. What You’ll Need to Have Four (4) year college degree in Dietetics or a nutrition‑related degree and preferred two (2) years experience in community or clinical nutrition education programs or completion of a dietetic internship with RD/RDN credentials Must hold current credentials as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration Bilingual (English/Spanish) strongly preferred Travel to multiple locations may be requested What You’ll Be Doing Day to Day Use the nutrition care process to conduct comprehensive nutrition assessments, recommend therapy interventions, and document nutrition care plans in the electronic health record Provide nutrition education and health promotion to clients and staff from different grants/programs, and develop preventive strategies for the Health Education team Clearly communicate and coordinate patient information among the care team to maintain proper care plans Evaluate and educate patients on nutrition‑related medications, dietary supplements, and drug‑nutrient interactions Develop, write, and review written nutrition‑related patient education materials and educational guidelines according to the local standard care plan Educate Health Education department staff on nutrition education guidelines Refer to appropriate interprofessional team members, resources, or programs as needed Assist in completing appropriate reports and conduct quality assurance according to federal guidelines Participate in Health Education team meetings, designated Specialty Team meetings, and quality improvement initiatives May develop relationships with external healthcare organizations to find additional resources Collaborate with the Health Education team in data collection, health outcomes reporting, clinical audits, and programmatic evaluation Help evaluate clinical care, resource utilization, and develop new clinical tools, forms, and procedures Attend trainings and conferences for continued professional development Use excellent customer service and communication skills Affirmative Action and Equal Opportunity Employer Adelante Healthcare is an affirmative action and equal opportunity employer.
