Description: Pain Symptom Management: Assess patients and administer medications/treatments to ensure comfort and alleviate suffering. Care Planning: Develop and update customized patient care plans in collaboration with physicians, social workers, and other team members. Family Support: Educate family caregivers on disease progression, what to expect, and medication management. Crisis Intervention: Respond quickly to sudden changes in a patient's condition to manage pain and provide crisis support. Documentation: Keep accurate, up-to-date patient medical records and facilitate communication between the care team.
2 years Hospice experience
Description: One week total of evening and weekend call per month
Certifications: BLS (AHA) StateLicenses: Michigan
