Home Health Aide- PCSP, LNA, HHA - Dunbarton, NH

Job Summary:

As a Home Health Aide (HHA) on our team, you will play a vital role in delivering high-quality care to our patients in their own homes.

Our care philosophy is to be part camp counselor and part crossing guard. Companion and build relationships while keeping our precious cargo clients safe.

Responsibilities:

• Provide personal care and assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting

• Assist with meal preparation, feeding, and light housekeeping tasks

• Monitor and report any changes in patient condition to the healthcare team

• Assist with medications and treatments as directed by the patient's plan of care

• Maintain a safe and clean environment for patients and provide emotional support and companionship

• Collaborate with other healthcare professionals to ensure seamless care delivery

• Document accurate and timely patient information in our electronic health record system

Requirements:

• High school diploma or equivalent required

• Currently certified as a Home Health Aide, PCSP, LNA

• 2 years of experience as a Home Health Aide

• Ability to work independently and as part of a team

• Ability to write care notes and clock in and out of Axiscare

• Strong communication and interpersonal skills

• Ability to transfer patients safely

• Reliable transportation and valid driver's license (if applicable)

• Must pass a fingerprint check in NH
