Job Summary:
• Outpatient Clinic
• Patient panel - Primary Care - Geriatrics

Essential Duties and Responsibilities:
• Coordinates the multidisciplinary care delivered
to their panel of patients, to optimize patient care.
• Reviews patient medical records to diagnose, treat, and provide care management and education to patients and families.
• Makes appropriate patient referrals.
• Champion a culture of empathy, dignity, and respect for aging populations.
• Monitors national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for the management of patient care.
• Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Demonstrates patient centered and patient valued behavior.
• Maintains a collaborative environment with other referring physicians, residents, medical students, advanced practice professionals, staff, and support staff to provide quality patient care.
• Supports academic, educational and research endeavors.
• Assess and manage chronic and acute conditions in patients with multiple comorbidities.
• Partners and communicates with acumen to build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Multispecialty Group. This would include health network staff, vendors, outside partners, patients, stakeholders, and local community leaders.
• Serves as a resource to the community.
• Performs other job-related duties as assigned.

Qualifications:

Required Qualifications:

Required Education and Experience: - Internal Medicine or Family Medicine residency and fellowship in Geriatric Medicine completion from accredited ACGME programs.
-Board certified by ABFM or ABIM required. Will consider candidates who are working towards CAQ in Geriatric Medicine within 5 year of joining.

Required Licensure/Certification/Specialized Training:
• Current, unrestricted medical license to practice in the State of Texas.
• Board Certification or Board Eligible.
• Unrestricted work authorization to work in the United States.

Preferred Education/Licensure/Certification and Experience: • Bilingual or multilingual proficiency is a plus.
• Prior experience in post-acute or long-term care settings strongly preferred.
• Proficient with electronic medical records and knowledgeable of CMS/SNF regulatory requirements. Key Competencies:
• Must be comfortable working in a large public/county facility
• Must be able to demonstrate a working knowledge in area assigned, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area
• Must have good interpersonal, verbal and oral communication skills to work effectively with other health care professionals and serve as a resource to the community
• Must be self-directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area
