Role Overview Provide skilled physical therapy in patients' homes under physician orders. You'll assess, treat, and educate patients and families while helping improve mobility, function, and quality of life.

Key Responsibilities

• Complete evaluations and OASIS assessments
• Develop and adjust individualized care plans
• Provide skilled treatment and patient education
• Monitor progress and communicate with care teams
• Supervise PTAs and coordinate care as needed
• Complete timely, accurate documentation per Medicare/agency guidelines
• Support continuity of care and reduce hospital readmissions

Requirements

• Active Physical Therapist license (state of employment)
• 1+ year acute care PT experience required
• Home health experience preferred
• Strong communication, documentation, and clinical judgment
• Valid driver's license + auto insurance
