Responsibilities

• Provide compassionate nursing care to hospice patients in accordance with individualized care plans
• Assess patient physical, emotional, psychosocial, and spiritual needs
• Administer medications, treatments, and symptom management interventions
• Monitor patient conditions and communicate changes to the interdisciplinary care team
• Educate patients and families regarding disease progression, symptom management, and end-of-life care
• Provide emotional support and guidance to patients and loved ones
• Collaborate with physicians, social workers, chaplains, therapists, and other healthcare professionals
• Develop and update patient care plans based on ongoing assessments
• Maintain accurate and timely documentation in accordance with regulatory and facility requirements
• Ensure compliance with hospice care standards, patient safety protocols, and quality initiatives
• Participate in on-call responsibilities as required by the facility

Requirements

• Active RN License in Virginia
• Minimum 2 years of recent nursing experience, preferably in Hospice, Home Health, Palliative Care, Oncology, or Med Surg
• Current BLS Certification required
• Hospice and Palliative Care experience preferred
• Strong assessment and symptom management skills
• Excellent communication, counseling, and patient advocacy abilities
• Ability to work independently and manage multiple patient needs
• Compassionate, patient-centered approach to care
• Strong documentation and organizational skills
• Commitment to providing high-quality end-of-life care
