Nurse Practitioner

About the Role Signify Health, part of CVS Health, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The clinician will conduct visits lasting about 45 minutes, including medication review, physical assessment, and any diagnostic tests requested by the health plan. Responsibilities Use an iPad and the clinical workflow to guide each evaluation. Provide diagnostic and preventive services such as spirometry, diabetes eye exams, and bone density screening. Maintain patient connections and guide them toward appropriate care. Be flexible to travel locally and within licensed states; conduct virtual visits as needed. Do not prescribe medication, order lab tests, or alter treatment regimens. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses preferred. Ability to cover primary locations in Vancouver, WA and surrounding area. Compensation and Benefits Weekly hours: 29.5 (part‑time).
