About the Role: As a Licensed Vocational Nurse (LVN), you will play a critical role in delivering compassionate and high-quality nursing care to patients in a post-acute care setting. Your primary focus will be to support the healthcare team by providing direct patient care, monitoring patient conditions, and administering prescribed treatments and medications. You will be responsible for maintaining accurate patient records and communicating effectively with patients, families, and interdisciplinary team members to ensure continuity of care. This role requires a strong commitment to patient safety, adherence to clinical protocols, and the ability to respond promptly to changes in patient status. Ultimately, your work will contribute to improving patient outcomes and enhancing the overall patient experience within the facility. Minimum Qualifications:
Skills: The required skills enable the LVN to provide safe and effective nursing care by accurately assessing patient conditions and administering treatments as prescribed. Strong communication skills are essential for coordinating with healthcare team members and educating patients and families, ensuring clarity and understanding. Attention to detail and organizational skills are used daily to maintain precise patient records and comply with regulatory standards. Preferred skills such as experience with EHR systems enhance efficiency in documentation and information sharing. Additionally, specialized certifications and multilingual abilities improve the quality of care and patient engagement, particularly in diverse or complex care environments. - Current and valid Licensed Vocational Nurse (LVN) license in the state of employment.
- High school diploma or equivalent required; completion of an accredited vocational nursing program.
- Basic Life Support (BLS) certification.
- Demonstrated ability to perform nursing duties in a clinical setting with attention to detail and patient safety.
- Strong communication skills and ability to work collaboratively within a multidisciplinary team.
- Experience working in a post-acute care, long-term care, or rehabilitation facility.
- Additional certifications such as IV therapy or wound care specialty.
- Familiarity with electronic health record (EHR) systems.
- Experience in patient education and discharge planning.
- Ability to speak a second language relevant to the patient population served.
- NOC Shift: 11:00 PM - 07:00 AM
- Provide direct nursing care to patients, including administering medications, wound care, and other treatments as prescribed by physicians.
- Monitor and document patients' vital signs, symptoms, and overall condition, reporting any changes to the supervising nurse or physician promptly.
- Assist with patient hygiene, mobility, and daily living activities while promoting patient comfort and dignity.
- Collaborate with the healthcare team to develop and implement individualized care plans tailored to each patient's needs.
- Maintain accurate and detailed patient records in compliance with facility policies and regulatory requirements.
- Educate patients and their families on health management, medication usage, and post-discharge care instructions.
- Ensure compliance with infection control protocols and safety standards to maintain a safe care environment.
