Germantown, TN Clinical Team Manager-RN - Hope Hospice & Palliative Care

JOB SUMMARY:
The Clinical Team Manager (RN) is responsible for the daily clinical operations and oversight of
an assigned patient care team, organized by geographical areas (zip codes) and assisting the
Director of Operations. This role ensures the delivery of high-quality, compliant, and
compassionate hospice care and ensures that all care meets or exceeds compliance with state,
federal and corporate guidelines. The Clinical Team Manager will supervise Case Managers and
field staff, monitor clinical documentation, and actively mentor team members while maintaining
the core values and mission of Hope Hospice. This position is responsible for the daily real time
quality improve daily functions within Hope.
QUALIFICATIONS:
1. Graduate of an accredited school of nursing.
2. Must be currently licensed (in good standing) in the state where the Hospice Program is
located.
3. At least one year of leadership, supervisory, or preceptor experience in a hospice or
related healthcare organization.
4. Knowledge of business management, federal and state regulations.
5. Deep understanding of hospice philosophy, regulations, and care delivery.
6. Strong interpersonal, leadership, and mentoring skills.
7. Proficiency in EMR systems and documentation review.
8. Strong critical thinking, organization and decision-making abilities.
9. Ability to build trust and maintain strong working relationships with staff and families.
10. Speaks, writes, reads, and comprehends English.
11. Good communication and interpersonal skills.
