Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals at Onyx Care of Natchitoches. As an LPN at our facility, you will provide high-quality patient care and support our team in delivering exceptional post-acute rehabilitation services.
Responsibilities:
Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and other treatments as prescribed by physicians
Develop and implement individualized care plans for patients, focusing on their unique needs and recovery goals
Collaborate with interdisciplinary teams to ensure seamless transitions and optimal patient outcomes
Monitor and report patient progress, identifying areas for improvement and making recommendations for modifications to care plans
Maintain accurate and detailed records of patient care, treatment, and progress
LPN will also perform other duties as assigned by the Charge Nurse or Nurse Manager
Requirements:
Current LPN license in the state of Louisiana
0 years of experience (Entry-level candidates welcome to apply)
LPN certification preferred
Ability to lift, transfer, and reposition patients
Strong communication and interpersonal skills
