Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals at Onyx Care of Natchitoches. As an LPN at our facility, you will provide high-quality patient care and support our team in delivering exceptional post-acute rehabilitation services.

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and other treatments as prescribed by physicians

• Develop and implement individualized care plans for patients, focusing on their unique needs and recovery goals

• Collaborate with interdisciplinary teams to ensure seamless transitions and optimal patient outcomes

• Monitor and report patient progress, identifying areas for improvement and making recommendations for modifications to care plans

• Maintain accurate and detailed records of patient care, treatment, and progress

• LPN will also perform other duties as assigned by the Charge Nurse or Nurse Manager

Requirements:

• Current LPN license in the state of Louisiana

• 0 years of experience (Entry-level candidates welcome to apply)

• LPN certification preferred

• Ability to lift, transfer, and reposition patients

• Strong communication and interpersonal skills
