Home Health Field Registered Nurse (RN)

Overview 21st Century Home Health Services (21HHS) serves over 4,000 patients across the Bay Area and is expanding into Marin and Sonoma counties. This entry‑level Field Registered Nurse position focuses on developing core nursing skills and supporting case management in home settings. All opportunities require regular field visits to patients’ homes. Duties and Responsibilities Providing hands‑on patient care within RN scope, including PICC lines, wound VAC and care, feeding pumps, NG, G, and J tube management, staples and suture removal, drain care (JP, biliary, nephrostomy), Foley/suprapubic catheter care, PleurX drainage, tracheostomy care, blood draws, ostomy management, injections, IV antibiotic or TPN administration, and compression wraps. Providing direct patient care as defined in the state Nurse Practice Act. Implementing a plan of care initiated and directed by the Case Management Registered Nurse. Documenting accurately and timely within 24–48 hours consistent with the plan of care. Assessing and educating patients, families, and caregivers about diagnoses and plan of care. Assisting the RN in performing skilled nursing procedures and duties, including equipment preparation and teaching self‑care techniques. Coordinating home health services and reporting needs for other disciplines (HHA, OT, PT, MSW, ST) to the Case Management RN or other disciplines. Using equipment and supplies efficiently and effectively. Participating in personal and professional growth and development. Performing other duties assigned by the Case Management Registered Nurse. Retrieving communication on assigned patients and using it to prioritize care. Providing timely and pertinent reports to the Case Manager. Recording concise information reflecting patient needs, problems, capabilities, limitations, and responses to interventions. Documenting incidents that affect patient care accurately. Reporting pertinent information promptly to the Case Management RN and providers. Attending weekly Case Conferences to coordinate and communicate patient care and treatment information. Ensuring patient privacy and maintaining medical records during care. Maintaining professionalism and seeking opportunities to enhance clinical skills. Meeting productivity standards as directed by the Case Management RN. Completing and reviewing Start‑of‑Care assessments, documentation, and plans of care with the Case Manager as needed. Participating in on‑call duties as required by weekend rotation. Job Specifications One year of recent nursing experience (waived for internal candidates or new graduates). One year of home health care experience preferred. Experience with high‑acuity patients is a plus. Experience working with an interdisciplinary team is highly desired. Graduate of an accredited nursing school (Bachelor’s degree). Current California Registered Nurse license (RN). Current Basic Life Support (BLS) card. Valid California driver’s license with an acceptable driving record. All licenses and certifications must be current at hire and maintained throughout employment. Self‑directed, critical thinker, outstanding interpersonal skills, organized, computer‑literacy, excellent observation, verbal and written communication, problem‑solving, basic math skills, and nursing skills per competency checklist. Must maintain annual compliance training and certifications. Compensation and Benefits Salary: $130,000–$160,000 per year, based on experience.
