Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Seeking an experienced LVN/LPN for a long-term care facility. The facility offers rehabilitation therapies, skilled nursing, memory care, and wound care. The night shift is slower-paced and requires strong attendance and punctuality. The nurse will administer medications and treatments as prescribed. Responsibilities include monitoring and reporting changes in resident condition. Timely and accurate documentation is essential. The nurse will provide nursing care following facility policies. Supervision of CNAs on assigned units is expected. Candidates must be organized, detail-oriented, and possess excellent communication skills. The role requires the ability to work with individuals from diverse cultures and personalities. A team player with a positive attitude is essential. Dependability and consistent attendance are crucial. Flexibility with scheduling needs is required. This role involves working with an Alzheimer's and dementia patient population. Candidates must be able to pass a medication administration test and clinical observation. Weekends are required on an every-other-weekend basis. RTO requests will only be approved at the time of submission and cannot be changed after an offer is made. Call-outs must be communicated with ample notice, more than 24 hours prior.

Minimum of 1 year of Long-Term Care (LTC) experience required. Must have a valid NY State License. BLS certification required. Must provide proof of license at the time of submission.
