Duties

Help At St. Cloud VA Health Care System, Registered Nurses (RN) in Mental Health Acute Psychiatric care plan, integrate, and deliver high-quality, timely care for Veterans-educating, counseling, and collaborating with teams to support safe transitions and stronger outcomes across the continuum of care. The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. Major duties include, but are not limited to:

• Planning, integrating, implementing, modifying, and evaluating the effectiveness of the Veteran's care.
• Serving as a liaison to internal and external providers and managing the care of the Veteran and the seamless transition to and from the community.
• Following the standardized care coordination model, incorporating case management principles, and processes related to referral, authorization, and secure medical documentation exchange.
• Identifying Veterans requiring disease management and obtaining resources internal or external to the U.S. Department of Veterans Affairs (VA), as applicable.
• Monitoring, tracking, and intervening, if necessary, in the episode of care across the continuum.
• Collaborating and co-managing the Veteran's care with the multidisciplinary team in a cost-effective manner.
• Providing care and counseling, health teaching, health counseling, case finding, and executing medical regimens including administering medications and treatments prescribed by authorized providers.
• Actively participating in unit/program level quality improvement processes, initiatives, and customer service programs.
• Demonstrating understanding and use of the Electronic Health Record and Bar-Coded Medication Administration (BCMA) software per policy.
• Participating in nursing service committees and in High Reliability Organization (HRO) processes and HRO initiatives.

Compensation

Day/Night rotation 7:30am-4:00pm, Midnight-8:00am, rotating holidays and weekends.
