Work Environment

This position is based onsite at our state-of-the-art Rehabilitation Facility located in Freehold, NJ, where our team members are encouraged to learn, share, and refine their expertise in physical therapy. Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing medical histories and performing detailed assessments to formulate individualized treatment plans.
• Communicate evaluation findings and treatment strategies to patients and their families, creating shared goal-setting initiatives to engage them in their recovery journey.
• Implement evidence-based treatment protocols tailored to each patient's unique presentations while adapting to evolving healthcare best practices.
• Collaborate with healthcare professionals, recognizing shifts in patient conditions that necessitate timely adjustments to care plans or consultations with specialists.
• Facilitate discharge planning, ensuring proper follow-up care is established and maintaining thorough, accurate documentation of patient progress and treatment outcomes.
• Exemplify effective time management and maintain productivity standards, ensuring timely attendance at meetings and active participation in departmental objectives.
• Engage in team efforts to eliminate barriers to accessing care, utilizing key performance data to inform decisions and initiatives aimed at improving patient access.
• Initiate and participate in professional development opportunities and serve as a clinical instructor for physical therapy students, contributing to the educational growth of the profession.

Patient Population / Client Focus

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.

Preferred Experience

Compensation and Benefits
