We are looking for a dedicated and skilled Full-Time Home-Based Physical Therapist to join our vibrant team at Mend Force Physical Therapy in Waldwick, New Jersey. This position offers a competitive annual salary ranging from $110,000 to $120,000, with a Monday to Friday schedule from 9 AM to 5 PM.

Work Environment

As a Home-Based Physical Therapist, you will primarily work onsite, traveling directly to patients' homes within the community. Key Responsibilities

• Conduct comprehensive assessments of patients' physical abilities, limitations, and needs using interviews, clinical observations, and physical testing protocols.
• Design and implement individualized physical therapy treatment plans that reflect patient-specific goals, clinical insights, and evidence-based practices.
• Deliver a range of therapeutic interventions including therapeutic exercises, manual therapy techniques, functional training, and balance improvement strategies tailored to each patient's unique situation.
• Educate patients and their families on the recovery process, pain management strategies, and recommend exercises to continue outside of therapy sessions, thereby fostering long-term health maintenance.
• Maintain meticulous and timely documentation of patient evaluations, treatment plans, progress notes, and outcomes according to regulatory standards and company policies.
• Monitor patient progress throughout therapy and adapt treatment plans accordingly, promptly reporting any changes in condition to both the physician and the patient's support network.

Patient Population / Client Focus

This position entails working closely with a diverse patient population, including individuals recovering from surgery, those with chronic care needs, older adults, and patients with neurological conditions. You will use your skills to help these individuals increase mobility, improve strength, and navigate daily activities with greater ease, thus enhancing their ability to lead fulfilling lives.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• Active and valid Physical Therapy license in the state of New Jersey.
• A minimum of 2 years of experience in physical therapy, preferably in homecare or outpatient settings.
• Strong clinical skills in assessment and manual therapy, accompanied by an advanced understanding of rehabilitation modalities.
• Valid driver's license and reliable transportation to travel to various patient homes.
• CPR and First Aid certifications are preferred; additional certifications in specialties such as geriatrics or neurological rehabilitation are a plus.
