We are seeking a Registered Nurse/Delegating Nurse/Case Manager to join our team! Come work in one of the most rewarding fields in nursing. We currently have a position to work with adults with intellectual disabilities at our Hagerstown, MD location. Position is eligible for full time benefits. Total of 40hrs per week, M-F day shift.

Responsibilities:

• Assessments for assigned caseload
• Development of and training for Nursing Care Plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Delegate and serve as resource to Direct Support Professionals/Certified Medication Technicians
• Educate patients/Direct support professionals about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Qualifications:

• Valid MD Nursing License or Compact State License
• Previous experience with intellectual/developmental disabilities preferred but not required
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a case management environment
