Registered Nurse (RN) – Home HealthWarm Hearth at HomeLocation: NRVJob Type: Full-TimeReports To: Clinical ManagerAbout the PositionWarm Hearth at Home is seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team.The Home Health RN provides skilled nursing care to patients in the comfort of their own homes and communities. This position is responsible for assessing patient needs, developing and implementing individualized plans of care, coordinating services, educating patients and families, and communicating with physicians and other members of the healthcare team.The ideal candidate is an experienced nurse who enjoys building relationships with patients and families and is comfortable working independently while being supported by a collaborative home health team.What You'll DoComplete comprehensive initial assessments of patients and families to determine home health needs.Perform complete physical assessments and obtain current and past medical histories.Regularly reassess patients and identify changes in their nursing needs.Develop and initiate individualized plans of care based on patient assessments and nursing diagnoses.Revise plans of care as the patient's condition and needs change.Provide skilled nursing care in accordance with the patient's plan of care and the Virginia Nurse Practice Act.Administer medications and treatments as prescribed by the physician.Perform appropriate preventive and rehabilitative nursing procedures.Educate and counsel patients and families regarding health, nursing care, and related needs.Provide patients and caregivers with appropriate health-care instructions.Identify discharge planning needs and implement appropriate interventions prior to discharge.Coordinate care with physicians, community health professionals, patients, and families.Communicate changes in patient condition and needs to the physician and obtain or receive physician orders as required.Prepare accurate clinical documentation and maintain timely clinical notes.Serve as Case Manager when assigned by the Clinical Supervisor and coordinate care for the assigned caseload.Coordinate necessary equipment, supplies, and services for patients.Instruct, supervise, and evaluate Home Health Aide services as required.Participate in on-call responsibilities in accordance with organizational policy.What We're Looking ForRequired:Graduate of an accredited school of nursing.Current Registered Nurse license in the state of Virginia.Current CPR certification.Strong assessment, clinical judgment, and problem-solving skills.Excellent verbal and written communication skills.Ability to work independently and effectively manage patient care in the home setting.Ability to meet the physical requirements of the position, including walking, standing, reaching, bending, kneeling, and assisting with lifting, positioning, and transferring patients.Valid driver's license.Reliable automobile in good working condition with insurance meeting organizational requirements.Preferred:1–2 years of recent acute care nursing experience in an institutional setting.Bachelor's degree in nursing.At least 1 year of home health experience.Why Home Health?Home health nursing allows you to provide meaningful, one-on-one care while helping patients remain safely and comfortably in their homes.
