Nurse Practitioner / Physician Assistant – Skilled Nursing & Long-Term Care

Location: Eugene, OR
We are seeking a compassionate Nurse Practitioner (NP/ARNP) or Physician Assistant (PA) to provide medical care in Skilled Nursing and Long-Term Care settings.

Responsibilities

Assess, diagnose, and treat geriatric and post-acute patients
Develop and manage treatment plans
Prescribe medications and interpret labs/tests
Provide post-hospital follow-up
Document care in the EMR
Collaborate with physicians and interdisciplinary teams
Ensure regulatory and quality compliance

Requirements

Graduate of an accredited NP/ARNP or PA program
Current Oregon license and national certification
SNF/LTC, geriatrics, post-acute, or primary care experience preferred
Strong clinical, communication, and organizational skills
