These team members support the mission of Hospice of Lenawee through their contribution to holistic health by providing spiritual care for Hospice of Lenawee through direct intervention and in cooperation with local religious leaders. The Spiritual Care Coordinator facilitates and provides spiritual care to patients and families working in collaboration with the interdisciplinary team. Spiritual Care Coordinator (Part-time, Day Shift) Pay & Benefits: Salary: Competitive, commensurate with experience Work-life harmony Generous Paid Time Off 11 Paid Holidays including your birthday Mileage Reimbursement Day Shift: 24 hours per week (some flexibility) Requirements: Graduate of an accredited seminary or school of theology or holds a BD, MDiv, or equivalent theology degree which qualifies him/her to serve as an ordained minister or certified lay minister with a specilty in pastoral care or counseling Endorsement or ordination from a particular religious body Has completed one unit of Clinical Pastoral Education (CPE) Valid driver license and vehicle with current insurance Experience in providing spiritual care in a clinical or parish setting, required Experience with dying patients and their families, hospice experience preferred Conducts initial and ongoing spiritual assessments by making appropriate home visits and contacts as determined by patient/family and documents all contacts in medical record. Makes regular spiritual care visits and contacts in compliance with the plan of care Provides spiritual care, counseling, and support to patients, and families and debriefing for staff Participates in interdisciplinary team meetings to share information about the patients' spiritual needs and concerns Offers to arrange contact with religious leader of patient/family preference and maintains a resource group of religious leaders of various denominations to whom care may be delegated Discusses funeral arrangements or other means of closure with patients and family members Available to conduct or participate in activities, memorial, religious, and prayer services as requested Available to educate religious leaders and other community groups about hospice and the spiritual concerns of persons experiencing accident, illness, or loss.
