Hospice Social Worker - LMSW/LBSW - Full-Time

About the Job

We are hiring a full-time, compassionate social worker (LMSW or LBSW) to provide hospice social services in and around the area! If you are looking for a career with a purpose and a supportive team culture, then this is the position for you!

Our Social Workers are responsible for providing a range of social work services to hospice patients and their families. Our Social Workers provide social service counseling and guidance to facilitate the maximum functioning and coping capacity to each patient and caregiver/family member.

Responsibilities:

• Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the RN Case Manager and other members of staff, including the interdisciplinary group.
• Assess the relationship between the patient's medical and nursing requirements and the patient's home situation, financial resources, and availability of community resources, including funeral arrangements if needed.
• Supervises the implementation of an individualized social work plan of care and notifies the Bereavement Coordinator of bereavement assessment needs.
• Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of psychosocial stressors and the symptoms of terminal illness.
• Counsels patient and family/caregivers as needed regarding stress and other identified coping difficulties.

Qualifications:

• Master's of Social Work from an accredited school of social work or Bachelor's of Social Work.
• Holds a current unencumbered license in Social Work in Texas.
• Minimum one (1) year experience as a social worked in long term care or medical surgical/acute care setting.
• One (1) year of hospice experience preferred but not required.
• Must be able to work flexible hours and travel between offices, facilities, etc.
• Must be computer proficient in typing and various programs, including a background in EMR.
• Must have the ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death, and grief.
• Possesses and maintains current CPR certification.
