Physical Therapist Assistant – Home Care & Hospice Location: North Conway, NH Schedule: Flexible schedule: you choose 3, 4, or 5 days per week. About the role The Physical Therapist Assistant supports patients in their homes by implementing individualized therapy plans created by the supervising Physical Therapist. This role focuses on restoring mobility, improving functional independence, reducing pain, and enhancing quality of life for patients receiving home care services or hospice care. You’ll work oneonone with patients, helping them stay safe, comfortable, and as independent as possible in their preferred environment. What You’ll Do Provide therapeutic interventions — Deliver exercises, gait training, strengthening, balance activities, and mobility support per the PT’s plan of care. Monitor patient progress — Observe, document, and report changes in condition, functional ability, and response to treatment. Educate patients & caregivers — Teach safe mobility techniques, fallprevention strategies, and home exercise programs. Support hospice goals — Focus on comfort, positioning, gentle mobility, and qualityoflife interventions for hospice patients. Collaborate with the interdisciplinary team — Communicate with PTs, nurses, aides, and social workers to ensure coordinated care. Maintain accurate documentation — Complete timely, compliant visit notes in the EMR. Ensure safety in the home — Identify environmental risks and recommend modifications to support mobility and independence.
