Service Facilitator (CCS)

We are seeking a compassionate Service Facilitator to support individuals with mental health challenges through a person-centered, recovery-focused approach. Responsibilities Include Conducting functional assessments and developing individualized service plans in collaboration with clients, families, and providers. Coordinating and authorizing community-based services and facilitating recovery-oriented team meetings. Connecting clients to resources, maintaining accurate documentation, and collaborating with treatment teams and community partners. Qualifications Master's degree in human services-related fields such as Social Work, Psychology, Counseling, or related disciplines. Valid driver’s license, reliable transportation, and ability to travel within the community. Ability to work flexible hours, including occasional evenings. Preferred experience in mental health, behavioral health, case management, or community services, especially with diverse populations. Key skills: care coordination, trauma-informed and recovery-oriented service delivery, documentation, teamwork, and community engagement.
