How you’ll make an impact in this role

• Deliver Compassionate Patient Care: Implement and actively monitor individualized care plans to keep patients safe, comfortable, and informed throughout their treatment journey.

• Coordinate Multidisciplinary Care: Lead cross-functional collaboration across care teams to ensure seamless communication, prompt execution of medical orders, and holistic patient support.

• Track and Communicate Progress: Monitor vital patient conditions and advocate for patient needs through clear, timely updates to families, physicians, and care team members.

• Champion Seamless Transitions: Assess patient needs early to coordinate comprehensive discharge plans that ensure smooth, safe transitions from hospital to home or continuing care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
• Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
