What You’ll Do Lead comprehensive nursing assessments and care planning. Manage a home health caseload across homes and community settings. Perform OASIS visits and coordinate interdisciplinary care. Provide education, guidance, and clinical direction to patients and families. Collaborate with physicians and care teams to drive outcomes. What We’re Looking For 3+ years of RN experience, home health preferred. Strong clinical decision-making and assessment skills. Confidence working independently in a field-based role.
