Activity CoordinatorColorado Springs Pikes Peak Location - Colorado Springs, CO 80909 End Date 10/30/2026OverviewSalary Range $19.50 - $22.43 Hourly Position Type Full TimeDescriptionPOSITION SUMMARYThe Activity Coordinator supports participant engagement, independence, and quality of life within the Rocky Mountain PACE Day Center by planning and facilitating person-centered recreational, therapeutic, and social activities. Activities are designed to promote participants' physical, cognitive, emotional, and psychosocial well-being. As a member of the Interdisciplinary Team (IDT), the Activity Coordinator contributes to participant assessments, care planning, and ongoing evaluation to ensure activity programming aligns with individualized goals, preferences, and functional abilities.CORE COMPETENCIESParticipant Engagement - Adaptive Problem-SolvingPerson-Centered Care - Effective CommunicationActivity Facilitation - Organizational SkillsTeam Collaboration - Safety AwarenessBehavioral Observation - Professional AccountabilityESSENTIAL JOB FUNCTIONSReasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.Activity Planning & FacilitationPlan, schedule, and facilitate daily group and individual activities, including physical exercise, cognitive stimulation, arts and crafts, music, socialization, reminiscence, special events, and community-based programming.Design and adapt activity programming to accommodate participants with varying physical, cognitive, emotional, and behavioral health needs.Promote participant choice, engagement, and independence while fostering a welcoming, respectful, and inclusive Day Center environment.Coordinate theme days, special events, and approved community partnerships.Assist with participant escorting and community outings as assigned, ensuring safety and adherence to organizational and regulatory requirements.Participant Support & SafetyObserve and monitor participant engagement, mood, behavior, and safety during activities and Day Center programming.Provide cueing, redirection, and activity modifications to support participants with dementia, cognitive impairment, or behavioral health needs.Assist participants with mobility, transfers, and use of adaptive equipment in accordance with training, safety standards, and individualized care plans.Provide limited assistance with activities of daily living (ADLs) within the Day Center as needed and within scope of training.Interdisciplinary Team (IDT) ResponsibilitiesActively participate in IDT meetings and care conferences, contributing observations related to psychosocial functioning, activity participation, and engagement.Collaborate with nursing, therapy, social work, dietary, personal care, transportation, and other IDT members to ensure activity programming aligns with individualized care plans.Communicate changes in participant condition, behavior, or functional status promptly to appropriate IDT members.Document activity participation, observations, and relevant participant responses in the electronic health record in accordance with PACE, CMS, and state requirements.Program Operations, Training & ComplianceMaintain clean, safe, and organized activity spaces in accordance with infection control, safety, and environmental standards.Assist with inventory management and ordering of supplies needed for activity programming.Support coordination and oversight of volunteers participating in activity-related programming.Participate in required staff meetings, training, regulatory compliance education, and quality improvement initiatives.Adhere to all PACE, CMS, and state regulations, including participant rights, confidentiality (HIPAA), infection control, and workplace safety policies.Perform other duties as assigned.ORGANIZATIONAL (CORE RATING FACTORS)Demonstrates consistent support of the organization's Mission, Vision, and Core Values.Provides exceptional customer service to participants, families, staff, and community partners.Maintains discretion and confidentiality in accordance with HIPAA and organizational policies.Supports a collaborative, respectful, and professional work environment.Embraces organizational excellence through effective time management, efficiency, and participation in continuous improvement efforts.Adheres to and supports all organizational policies, procedures, and safety practices.Brings forward ideas, concerns, and issues and participates in resolution and implementation.Completes required regulatory and compliance training within prescribed timeframes.Attends required meetings and maintains competencies necessary for the role, including communication skills and basic computer proficiency.Managerial Breadth / Scope of JobThis position has no direct supervisory responsibilities but supports team members at all levels across the organization within scope.QualificationsEducation & ExperienceHigh school diploma or equivalent required; associate's degree in recreation therapy, social services, gerontology, or a related field preferred.Minimum of one (1) year of experience working with older adults, preferably in PACE, adult day programs, long-term care, or memory care settings preferred.Experience planning and facilitating activities for individuals and groups with varying physical, cognitive, and emotional needs.National certification in activity programming is preferred (e.g., NCCAP or another nationally recognized certification). Individuals who are not certified at the time of hire are required to obtain certification following employment. Certification must be initiated within 120 days of hire, with ongoing participation in required coursework and examinations each semester the coursework is offered. Certification must be fully completed no later than 12 months from the start of coursework.Current CPR and First Aid certification or ability to obtain upon hire.Valid driver's license and ability to meet agency driving requirements if assigned to outings or transportation.Ability to pass all required background checks, health screenings, and drug testing in accordance with PACE regulations.Knowledge & SkillsKnowledge of activity programming for older adults, including dementia-friendly and trauma-informed approaches.Ability to design and adapt inclusive, engaging activities for participants with diverse functional abilities.Strong interpersonal, communication, and teamwork skills.Ability to observe, document, and communicate participant engagement, behavioral changes, and functional status accurately and timely.Strong organizational and time-management skills with the ability to manage multiple activities and priorities.Ability to work collaboratively within an Interdisciplinary Team (IDT) environment.PHYSICAL DEMANDS AND WORKING CONDITIONSThe physical demands and working conditions described below are representative of those required to perform the essential functions of the job.This position requires frequent standing, walking, bending, kneeling, and assisting participants with mobility and transfers. The employee may be required to lift, push, or pull participants, supplies, or equipment up to 50 pounds using appropriate body mechanics or assistive equipment. The role involves working in a fast-paced Day Center environment serving frail older adults and may include exposure to communicable diseases, bodily fluids, unpleasant odors, and participants who may exhibit physical or verbal aggression.
