External Care Manager (ECM)

Complete Care is looking for an experienced and relationship-focused External Care Manager to join our growing team in Hackensack, New Jersey. This role serves as a key liaison between hospitals, physicians, discharge planners, families, and our skilled nursing facilities to help coordinate seamless transitions of care and drive census growth. The External Care Manager will represent Complete Care throughout the local healthcare community, develop referral relationships, and ensure patients and families receive exceptional support during the admissions process. Responsibilities Build and maintain strong referral relationships with hospitals, physicians, case managers, assisted living communities, and healthcare providers throughout the Hackensack market Conduct daily outreach and in-person visits to referral sources Coordinate admissions and facilitate smooth transitions into Complete Care centers Educate referral partners on Complete Care’s clinical programs, specialty services, and patient outcomes Collaborate closely with facility admissions teams, nursing leadership, and regional operations Track referral activity and maintain consistent communication with prospective patients and families Participate in community events, networking opportunities, and healthcare meetings Identify opportunities for census growth and market expansion Maintain knowledge of payer sources, insurance plans, and post-acute care services Qualifications Experience in healthcare marketing, admissions, case management, discharge planning, or business development preferred Skilled nursing, long-term care, rehab, or hospital experience strongly preferred Strong relationship-building and communication skills Ability to work independently while managing multiple priorities Knowledge of the Hackensack and Bergen County healthcare market is a plus Valid driver’s license and ability to travel locally Why Join Complete Care?
