Social Services Director Terraces of Lake Worth Care Center, a Skilled Nursing Facility located in Lake Worth, FL, is seeking a highly motivated and experienced Social Services Director to join our team! Lake Worth Beach, Florida $70,000 - $75,000 Annually Full-Time Responsibilities Promote transitions of care across the continuum Work with community resources to ensure patients have a safe plan when transitioning out of the facility Possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services Monitor preadmission to discharge practices to assure quality patient care and compliance with regulatory requirements Requirements Minimum of two (2) years of social work experience in a skilled healthcare setting working directly with individuals Strong understanding of community resources and the ability to coordinate plans for discharges Excellent communication and interpersonal skills What We Offer A comprehensive benefits package A dynamic and supportive work environment Opportunities for professional growth and development A commitment to providing high-quality care to our residents #J-18808-Ljbffr Terracescc
