PT - Physical Therapist

Outpatient Physical Therapist

This is a 13-week contract position.

Flexible schedule of 40 hours per week with 1-2 evenings until 7 PM.

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities.

Responsibilities:

• Complete comprehensive evaluations and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document patient care clearly, accurately, and timely in compliance with policies and procedures.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student programs as assigned, providing a positive learning environment with appropriate supervision.
• Create an individualized professional development plan and pursue continuous education and professional growth.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Engage in performance improvement initiatives to enhance the quality of services provided.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Valid state licensure as a Physical Therapist in good standing.
