Hospice Field Staff RN/Case Manager Hospice of Southern Illinois is a not‑for‑profit, free‑standing, community‑based hospice serving Southern Illinois and the St. Louis Metro‑East communities. As a member of the interdisciplinary team, the RN/Case Manager oversees the direction and coordination of the patient’s care throughout their time in hospice. Pay : $39.21 hourly. Full‑time non‑exempt, 40 hours weekly. Schedule : Monday‑Friday 8:30 AM‑5:00 PM with an additional rotational on‑call schedule that includes evenings, weekends, and holidays. Location : Based at the Belleville Patient Services Office, primarily visiting patients within the Belleville service area (St. Clair, Monroe, Randolph, Washington counties). Position Requirements Education Graduate of a state‑approved school of nursing. Associate Degree or Diploma in Nursing required. Experience One (1) year of hospice experience preferred. Other Qualifications Current Illinois nursing license. Certification in hospice and palliative care preferred. CPR certification preferred. Excellent leadership, managerial, clinical assessment, organizational and interpersonal skills. Working knowledge of Medicare/Medicaid, hospice regulations, and third‑party payor expectations. Reliable means of transportation. Physical Requirements Perform repetitive tasks—bathing, transferring, lifting, and turning patients; sit for prolonged periods; exert up to 100 lbs occasionally, 50 lbs frequently, and 20 lbs constantly. Requires flexibility to bend at the knee and squat‑bend. Must manage stress, make decisions under pressure, and handle multiple priorities. Requires clear vision within 20 inches to 20 feet, and hearing alarms, phone, and tape recorder. Working Conditions Approximately 20% of the time is spent in an air‑conditioned environment with variable humidity and noise; 80% in a patient’s home or automobile. Exposure to malodorous odors, infectious bodies, pet hair, allergens, noxious cleaning agents, communicable diseases, and traffic may occur. Essential Functions of the Field Staff RN/Case Manager Maintain a caseload of patients and oversee the direction and coordination of their care and that of family and caregivers. Provide direct patient care, including all aspects of the nursing process, and participate in on‑call after‑hours care. Demonstrate required registered nurse clinical skills and exhibit them at the annual skills day. Complete a physical assessment, medication review, and symptom/pain evaluation on each visit; enter medication orders in a timely manner; communicate findings to the hospice physician and obtain orders; order medications from pharmacy promptly. Communicate pertinent information to all disciplines via communicator, telephone, encrypted text messaging system, and at the IDT meeting. Review and update the Plan of Care on each visit. Initiate, supervise, and update the Hospice Aide (HA) plan of care in a timely manner; supervise visits at least every 14 days per Medicare regulation. Explain disease progression patterns and provide effective ongoing education to patients, families, and caregivers. Apply principles of end‑of‑life pain and symptom management, considering patient and caregiver values and goals to keep patients pain‑free. Identify physical, psychosocial, emotional, and spiritual needs related to terminal illness and collaborate with the IDT to meet those needs. Document assessment elements systematically and in a timely manner according to policies and performance standards. Oversee the activity of the Licensed Practical Nurse (LPN) providing patient care. Participate in on‑call after‑hour care, triaging calls and demonstrating critical thinking skills in prioritizing and addressing each situation. Respond to emergency or crisis events with control, care prioritization, and quick execution of a plan. Provide emotional, spiritual, and psychosocial support; establish a therapeutic relationship with patient, family, and caregiver. At the IDT meeting, at least every 15 days, share new information on disease progression, suggest improvements to quality of life and comfort, and ensure the plan reflects the patient’s core wishes. Work cooperatively with a team to meet deadlines, goals, and objectives. Embrace change, manage stress, and maintain a positive attitude. Build relationships, collaborate with internal and external customers, cultivate referrals, analyze referral/admission problems, and propose solutions for better customer service and satisfaction. Participate in QAPI activities and maintain compliance with performance improvement measures. Organize and work efficiently with minimal supervision; be a self‑starter demonstrating flexibility, motivation, initiative, and commitment. Identify and contribute to new ideas and approaches to improve work processes. Integrate Hospice of Southern Illinois core values into daily practice. Manage paid time off appropriately and submit online requests in a timely manner. Maintain accurate timesheet documentation according to policy and standards. Other duties as assigned.
