Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis, past medical history, precautions, and contraindications
• Establish measurable goals and develop treatment plans
• Regularly re-assess the effectiveness of treatment plans and modify as needed
• Communicate patient needs and progress to the treatment team and relevant parties
• Consult with medical providers regarding evaluations, progress, and treatment discharge
• Recommend and assist in securing special adaptive equipment for patients
• Maintain and submit required documentation in compliance with regulations
• Supervise Physical Therapist Assistants and Aides in accordance with state practice act
• Participate in and conduct staff development programs and maintain clinical practice skills

Requirements of the Physical Therapist

• Valid Physical Therapist license
• Experience in Home Health required
• HCHB experience preferred but not required
