Shelter House
Shelter House seeks a Medical Outreach Case Manager to support Kennedy Emergency Shelter clients. You will provide intensive health-focused case management, coordinate transportation to appointments, and connect individuals to a medical home with a trauma-informed, harm-reduction approach. Work occurs in nontraditional settings such as campsites and shelters; strong interpersonal skills and flexibility for evenings/weekends are essential. #J-18808-Ljbffr Shelter House
