POSITION SUMMARYPosition will provide professional medical services to patients in a clinic setting under the direction of a licensed physician, advanced nurse practitioner, or physician assistant. Position will assist in training of other Agency staff regarding blood borne pathogens, tuberculosis, first aid, and HIV Workplace Guidelines. Provides education and medical services for people with HIV disease and other healthcare conditions. Assists in providing medical case management services. Position provides technical assistance regarding tuberculosis activities. This position will assist the medical case manager and other senior staff in developing clinical services and accessing other funding sources.ESSENTIAL RESPONSIBILITIES:Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Vaccine Coordinator for the ClinicMonitors clients, following protocol as outlined in the standards of careProvides medication adherence education and appropriate interventions to each client as needed.Assists medical staff with all procedures and follows directivesScreens clients on clinic days, obtaining weight, vital signs and historyDraws labs, do UAs, check blood sugarsLogs and reports lab resultsEducates client regarding medications, adherence, safe sex and preventive health managementScreens medical related walk-ins for vital signs and severity, refers emergencies to ERSchedules clients to do routine labs and procedures ordered by physicianReviews and reports panic labs to medical supervisor as receivedScreens medical related phone calls, assessing client needsAdministers vaccinations, TB tests, and injections as orderedTracks clients TB testing and vaccinations for compliance to agency protocolUpdates and documents accurately in client chartsCommunicates with case managers and other staff to ensure quality client careCalls in prescriptions and refills to pharmacyPerforms patient follows ups and makes referrals as neededFollows agency protocol when handling biohazardous and infectious materialsOrders supplies for clinic as neededFills out state medical applications and medical certification formsCompletes and processes patient assistance forms for medicationsCodes and completes billing forms according to funding source requirementsPrepares confidential and non-confidential correspondence in a professional style and formatPrepares time reports and mileage accurately and on timeComplies with all HIPAA requirementsConducts community presentations as necessaryAttends all staff meetings and trainings as assigned by supervisor.Conducts self in a manner consistent with professional standards.Adheres to all Agency personnel and program policiesOther related duties as assignedQUALIFICATIONS:Licensed vocational nurse by the state of Texas or Licensed Practical Nurse by the State.Outpatient Clinic experience preferred.Insurable for professional liability.KNOWLEDGE, SKILLS, AND ABILITIES REQUIREDGood independent judgment skills.Excellent written and oral communication skills.Able to schedule work activities according to priority.Ability to complete documentation in legible and professional mannerNeeds to be familiar with community resources and be able to direct persons to the appropriate services.Ability to follow procedures while working in an independent environment.Must be self-starter and self-directive. #J-18808-Ljbffr Special Health Resources For Texas Incorporated
