You’ll work closely with patients and their care teams to promote health, independence, and quality of life. Your responsibilities may include: Providing wound care Managing and administering medications Providing catheter care Supporting diabetic patients with care and education Monitoring overall wellness and changes in condition Providing disease-management education Following up with patients after physician appointments Identifying and ordering necessary medical supplies Completing accurate and timely electronic documentation Home care offers a unique opportunity to get to know your patients and make a meaningful difference in their daily lives.
