Job ID View phone number on click.appcast.io JOB OVERVIEW The Wellness Nurse is responsible for monitoring the health and well‑being of residents and providing leadership support and assistance with resident care and services. Responsibilities include overall well‑being of residents, training and monitoring medication care administration, communication with resident support services and families, and maintaining clinical quality assurance while demonstrating Sunrise’s Mission to champion quality of life for seniors. RESPONSIBILITIES & QUALIFICATIONS As part of the Sunrise team, supporting our Mission, Principles of Service, and Core Values is fundamental. The role includes the following essential functions: Coordination of Health Needs Identify ongoing needs and services of residents to promote the highest‑quality resident services. Ensure proper follow‑through and participate in evaluation and documentation for residents with changes in health‑care services and status. Assist the Resident Care Director (RCD) in conducting resident evaluations and gathering data at move‑in, 30‑day post‑move‑in, semi‑annually, and when status changes. Conduct monthly wellness visits for all residents. Monitor and follow up with residents who have experienced a fall, accident, or incident; document and report as required. Document all pertinent information in the resident wellness file. Maintain communications with residents’ families or responsible parties to ensure medical needs are met. Contact residents’ attending physicians when necessary or upon family request. Ensure weights and vital signs are obtained monthly for each resident. Assist in maintaining wellness files per Sunrise policies and federal and state/provincial regulations. Maintain medical supplies and emergency kits for the community. Provide clinical support and assistance to community team members as needed. Understand and follow infection‑control practices. Partner with the RCD and community team to promote infection‑control standards, including resident testing, training, and skills checks. Comply with universal care precautions regarding blood‑borne pathogens, bodily fluids, hazardous waste, sharp sticks, and disposing of contaminated materials. Medication Programs Monitor each resident’s medication and treatment profile regularly to ensure accurate administration and documentation. Transcribe physician orders (MAR/TAR) with initials, date and time, and date faxed to pharmacy promptly. Complete MAR/TAR audit each month. Demonstrate knowledge of the six rights of medication administration. Act as liaison for pharmacy services to ensure effective resident services. Monitor medication room, carts, and treatment carts for neatness, cleanliness, availability, and expired medications. Complete medication observations and skills checks for medication care managers per RCD direction. Quality Assurance and Regulatory Compliance Demonstrate knowledge in key quality improvement areas: Resident‑centered care model APIE Quality care indicators and outcomes Active participation in Sunrise Quality Assurance and Clinical Meetings Demonstrate knowledge of key regulations: All federal, state/provincial, and local resident‑care regulations Resident rights Resident assessment and evaluation process per state/provincial requirements Review physician reports of new residents as directed by RCD. Order medications and equipment as directed by RCD. Demonstrate knowledge of Sunrise practices in skin, nutrition, behavior, falls, and bowel/bladder care. Assist in training and monitoring documentation related to clinical health and wellness. Report resident changes, concerns, or solutions to RCD as appropriate. Contribute to engagement programs and processes, including team‑member engagement survey participation and engagement improvement workshops. Attend regular meetings such as town‑hall, department team, cross‑over, and others per supervisor/department coordinator. Maintain compliance in assigned and required training, and all training required by state/provincial or other regulators. Perform other duties as assigned. Good judgment, problem‑solving, and decision‑making skills. Experience and Qualifications To perform successfully, an individual must be able to do the following: Graduate of an approved college/school of nursing. Maintain a current state/provincial licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN). Minimum of one year experience in home‑health, assisted‑living, hospital, or long‑term care environment. Knowledge of nursing practices, techniques, and methods applied to resident care and local regulations. Good assessment skills knowledge. Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Sunrise applications with ability to learn new applications. Availability for weekends, evenings, and flexible hours during peak service delivery times.
