At AdviniaCare Newton Wellesley, our residents aren't just names on a caseload. We specialize in long-term care and memory care, giving our therapists the chance to build meaningful relationships with residents, families, and an interdisciplinary team while helping people maintain independence, mobility, and quality of life. Success here isn't always measured by a discharge home; sometimes it's a safer walk to the dining room, fewer falls, or finding the one approach that finally connects with a person living with Alzheimer's or dementia.

We're growing our rehab team and looking for a full-time Physical Therapist who wants to practice differently one who values specialized dementia rehabilitation, creative clinical problem-solving, and person-centered, function-focused therapy. Applicants wanting part-time are also encouraged to apply!

Responsibilities

• Assess resident needs and develop individualized treatment plans
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Apply function-focused, person-centered approaches to support independence and mobility
• Collaborate with the interdisciplinary team to ensure coordinated, relationship-based care
• Educate residents and families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate, up-to-date patient care records

Requirements
