The Per Diem Advanced Practice Clinician joins Integrated Clinical Services, delivering professional, comprehensive nursing care across the Portland Metro Area. Department: Health Department. Job Type: On-Call. Key Responsibilities Diagnosing, managing, and treating acute and chronic medication conditions, including prenatal care; conducting medical history and physical examinations; ordering and interpreting diagnostic tests; devising treatment plans; prescribing and monitoring medications; counseling patients; and reassessing patients as appropriate. Utilizing panel‑based information technology tools to prioritize and direct proactive care for provider panel patients, including preventive health screening and chronic disease management. Providing follow‑ups on utilization issues such as ED visits and hospital admissions. Establishing the need for specialty and rehabilitative services and making appropriate referrals. Assessing and counseling patients regarding preventive care needs and delivering services or making appropriate referrals. Determining the need and scheduling for high‑risk referrals. Seeking clinical consultation from other providers as needed and providing consultation to nurses and support staff. Prioritizing and directing activities of the care team; coordinating with Behavioral Health providers to effectively integrate care. Providing coverage for clinical colleagues in their absence. Completing patient documentation and managing electronic in‑basket according to established policies and procedures. Participating in quality improvement and assurance projects as needed. Minimum Qualifications Master's Degree in Nursing (NP or PA) or equivalent. Three (3) to six (6) years of clinical experience. Licensed Nurse Practitioner or Physician Assistant in Oregon.
