CASE MANAGER (RN) - FULL TIME

Responsibilities Collaborate with patients, patient family members, physicians, and other healthcare professionals to design, implement, and evaluate individualized plans of care. Maintain high-quality, cost-effective patient care through effective plan implementation. Gather and exchange data internally and externally with key medical professionals, patients, and family members. Assist patients by communicating with community stakeholders to appropriately support their needs. Qualifications Bachelor’s degree preferred; RN licensure required (valid, current state license or compact). At least three (3) years of previous clinical and/or case management/utilization management experience, preferably in a high-volume acute care hospital or healthcare facility. Excellent interpersonal, communication, and problem‑solving skills. Strong critical thinking and problem‑resolution abilities. Ability to communicate effectively with community stakeholders.
