Hospice RN Case Manager - Home Care Team Lead

Constellation Health Services is seeking a compassionate Registered Nurse to provide hospice care to patients and support to their families in home, assisted living facilities, and skilled nursing facilities throughout Stafford, CT. You'll administer medications, assess patient needs, coordinate care with interdisciplinary teams, and document visits using our EMR.
