Our organization is looking for a dedicated and empathetic Registered Nurse (RN) to deliver skilled nursing services in the comfort of patients' homes throughout Western Colorado. This role involves performing thorough assessments, administering medications and treatments, managing wound care, and collaborating with a multidisciplinary team to ensure comprehensive patient care.

• Conduct detailed patient assessments to determine care needs.
• Administer prescribed treatments and medications effectively.
• Manage and treat wounds with precision.
• Develop personalized care plans tailored to individual patient requirements.
• Educate patients and their families about health management and care procedures.
• Document all patient interactions and care activities in an electronic medical record (EMR) system.
• Maintain open communication with physicians and other healthcare professionals.

• Current RN license in the state of Colorado.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently while also being a team player.
• Excellent communication and interpersonal skills.
