Hospice RN Case Manager - On-Call Home Care

Crossbridge Hospice in Troy, MI is seeking an experienced RN Case Manager to deliver end-of-life care in patients' homes and community settings. You will coordinate and supervise hospice services, educate staff, and partner with families to ensure compassionate, patient-centered care.
