Makes follow up visits according to patient, family, and facility needs./liliCommunicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care./liliRevises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members./liliCounsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care./liliTeaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.)./liliCollaborates/Communicates with Medical Director, attending physician, branch leadership and IDG./liliPerforms other duties/projects as assigned./li/ulpWhat is Required?/pulliGraduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing/liliCurrent State Licensee as a Registered Nurse RN/lili1 year of experience in a clinical care setting/liliExperience in hospice or home health environment is preferred/liliNursing skills as defined by accepted nursing standards/liliAbility to handle multiple priorities, documentation requirements, and deadlines/liliAble and willing to travel up to 50% as needed to support business and patient needs./liliAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment/liliValid drivers license and insurance and reliable transportation to perform job tasks/li/ulpYou will report to the Clinical Team Manager./ppEqual Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
