As a LPN or RN MDS Coordinator , you will play a vital role in ensuring the highest level of care for our residents. LPN or RN MDS Coordinator will be responsible for coordinating and implementing the MDS process, facilitating communication between care teams, and providing exceptional customer service to our residents and families.
