In this Community Health Nurse position, you will be responsible for assessing, planning, implementing, and evaluating public health services and initiatives, as well as providing direct public health services and community liaison activities. Your work will provide key support to our mission of providing infrastructure for the implementation of core public health functions in a high quality, effective, and responsive manner that will positively impact the health status of all people in Pennsylvania. Duties in this role include the following:
- Health Data Analysis: Prepare and analyze county health assessments to identify public health needs and resources
- Professional Collaboration: Represent DOH on district and local health committees; help develop public health initiatives; and liaise with community partnerships to promote awareness and provide consultation, technical assistance
- Community Partnerships: Demonstrate awareness of vulnerable populations and provide advocacy, outreach, and case management services to promote patient health and safety
- Program Monitoring: Monitor community health programs using public health information system databases to ensure improved health status for assigned programs and populations
- Public Education: Develop and provide health education programs on a variety of public health topics to educate the public
- Clinical Expertise: Provide clinical services and consultation; set up clinics, programs; transport clinical supplies; operate and transport equipment

Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
- Work infrequently requires statewide travel and irregular/flexible work hours including overnights, weekends and holidays in accordance with policy.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

Qualifications
Special Requirements:
- All positions require active authorization to practice as a Registered Nurse in Pennsylvania.
- All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
Other Requirements:

- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
